～October 2025; Results of the health checkup for international students（Honjo Campus）～
2025年10月29日（水）に実施された留学生健康診断を受診された方の『健康診断証明書』の発行が可能になりました。
健康診断証明書は、自動発行機で取得できます。健康診断の結果通知も兼ねています。学生の皆さんは必ず確認をしてください。尚、自動発行機で発行できない場合は、保健管理センターでの発行となります。
【発行できる場所】
本庄キャンパスは学生センターと、教養教育1号館1階学生ホールの自動発行機により交付します。自動発行機による交付は、当該年度の健診の必要項目をすべて受診し、異常の無い場合に発行可能です。自動発行機で発行できない学生は、保健管理センターで発行します。保健管理センター既定の用紙以外の申請および健康状態により、即日発行ができない場合がありますので注意してください。
【発行できる期間】
健康診断書証明書の発行は当該年度のみの発行で、前年度以前の健診についての証明書は発行できません。
Health Certificates are now available for those who underwent the international student health checkup held on Wednesday, October 29, 2025.
Health check certificates can be obtained from the automatic issuing machines. They also serve as notification of the health check results. Students should be sure to check. If the automatic issuing machine is unable to issue a certificate, it will be issued at the Health Management Center.
[Where to Issue Certificates]
At the Honjo Campus, certificates are issued at the Student Center and the automatic issuing machines in the Student Hall on the first floor of the Liberal Arts Education Building 1. Certificates can be issued from the automatic issuing machines only if all the necessary items for the medical checkup for that year have been taken and no abnormalities have been found. Students who are unable to get a certificate from the automatic issuing machines will have their certificate issued at the Health Care Center. Please note that same-day issuance may not be possible depending on the application form other than the Health Care Center’s designated form and the student’s health condition.

Health Certificates are only issued for the current academic year; certificates for health checkups from previous years cannot be issued.
